Position: Intensive Case Manager
Location: Cummins Behavioral Health Systems, Putnam County Office

Key Responsibilities:
· Provide comprehensive case management services to individuals with complex needs.
· Develop and implement individualized care plans in collaboration with clients and their families.
· Coordinate with healthcare providers, social services, and community resources to ensure holistic care.
· Monitor and evaluate client progress, adjusting care plans as necessary.
· Advocate for clients' needs and support them in achieving their goals.
Qualifications:
· Bachelor's degree in social work, psychology, or a related field required; Master's degree preferred.
· Experience in case management or a similar role.
· Strong organizational and communication skills.
· Ability to work independently and as part of a multidisciplinary team.
· Empathy, patience, and a commitment to client-centered care.
