The Mercer Center for Intellectual and Developmental Disabilities Care clinic in Macon, GA is searching for a Medical Social Worker. The candidate in this position under the supervision of the CSDC Nurse Manager, will provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or disabling illnesses. Services include advising family caregivers, providing patient education and counseling, and making referrals for other services. The social worker may also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to healthcare access. Qualifications include a Master's Degree in Social Work, two years of experience in a hospital or community-based social services program, licensure in Georgia as a Clinical Social Worker (LCSW), and valid CPR Certification. Bilingual Spanish/English and experience with Electronic Medical Records Systems (preferably Athena) are highly desirable. Knowledge and skills required include strong communication, expertise in conflict mediation, abuse and neglect, advance directives, crisis intervention, government programs, CMS and Medicaid regulations, and the ability to work independently and make sound judgments in complex situations. Background check including criminal history is required. Document attachments required are resume, cover letter, and list of three professional references.
