HOURLY RANGE: $30.68 - $41.57

DISTINGUISHING FEATURES OF THE CLASS: The work involves planning, coordinating and/or conducting of education, counseling, assessment and follow up activities for patients with health related diseases at the Erie County Medical Center Corporation. The incumbent provides a wide variety of educational, counseling and referral services to assist patients in receiving care and consultation as inpatients, outpatients and after discharge. The incumbent also assesses and evaluates the effectiveness of individual patient care programs. Work is performed under the general supervision of higher-level professional and administrative staff. Supervision is not a function of this position. Does related work as related.

TYPICAL WORK ACTIVITIES:

Develops plans for patient education and care program based on patient needs and co-diagnosis; reviews plans with patients and caregivers; Counsels patients on disease, lifestyle choices and behavioral modifications; Coordinates patient medical needs with medical and professional staff such as doctors, nurses, case managers and discharge planners; Researches and provides referrals for patients to utilize community health resources and outpatient services; Assists in scheduling patient follow-up appointments as needed; Maintains accurate patient data related to education plan, counseling and consultation with patients and caregivers; Conducts follow-up calls and interacts with patients after discharge to ensure patient needs are met and encourage compliance with lifestyle choices and behavioral modifications; Collects and maintains patient data; inputs patient data into appropriate electronic health applications; Provides information and prepares reports as requested; Performs client screening, assessment and evaluation using standardized and customized instruments; Develops and implements customized assessment and evaluation instruments and related written materials; Evaluates individual patient education and care program; may amend patient education and care program based on evaluation; Researches and implements updates to protocols and procedures related to compliance and patient care; communicates changes to appropriate medical and professional staff; Designs and implements evaluation tools to measure overall services provided to patient; performs evaluations; Develops and distributes evaluation findings and provides recommendations to improve services provided to patients; Identifies and notifies management of potential unsafe practices; Participates in departmental performance improvement activities; Serves on committees, attends and participates in meetings. FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Through knowledge of creating and presenting educational and care plans for patients and caregivers; thorough knowledge of the principles, practices and techniques of disease prevention; good knowledge of the methods and procedures used in the course of hospital care; good knowledge of creating patient care assessment and evaluation instruments; good knowledge of program evaluation; ability to recognize and identify symptoms; ability to coordinate medical treatment with allied disciplines; ability to respond to emergencies promptly and effectively; ability to compile, maintain and analyze patient and statistical data; ability to prepare reports; ability to establish and maintain effective working relationships with a wide variety of constituents; ability to communicate effectively, both orally and in writing; ability to use a variety of electronic software applications including Microsoft Office and electronic health/medical record systems; patience; compassion; sound judgment; neatness; tact; capable of performing the essential functions of the position with or without reasonable accommodations.

MINIMUM QUALIFICATIONS:

Possession of a Master's Degree* in Public Health, Health Education, Nutrition, Infection Control, Diabetes Education or health-related field and two (2) years of experience working in direct patient care with disease populations, educating on lifestyle modification and/or disease prevention. SPECIAL REQUIREMENT IF ASSIGNED TO CONGESTIVE HEART FAILURE AND DIABETIC MANAGEMENT: Possession of Diabetes Educator Certification from the National Certification Board for Diabetes Educators (NCBDE) within twelve (12) months of appointment and maintenance throughout duration of appointment. NOTE*: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.
